Sufentanil ist ein synthetisches Thienylderivat von Fentanyl mit außerordentlich hoher Potenz: ca. 5-10× potenter als Fentanyl und 500-1000× potenter als Morphin. Wie alle μ-Opioid-Agonisten hemmt Sufentanil über Gi-Protein-Kopplung die Adenylylcyclase (↓cAMP), aktiviert einwärtsgleichrichtende K⁺-Kanäle (Hyperpolarisierung) und inhibiert spannungsgesteuerte Ca²⁺-Kanäle. Dies führt zu Analgesie, Sedierung, respiratory depression und Euphorie. Extrem hohe Lipophilie (Oktanol/Wasser-Koeffizient >>Fentanyl) ermöglicht ultraschnelle ZNS-Penetration. Die Kombination aus ultrahöher MOR-Affinität, rapidem ZNS-Eintritt und vollständigem Agonismus ergibt einen marginalen Sicherheitsabstand: Der Unterschied zwischen analgetischer und letaler Dosis ist bei Sufentanil kleiner als bei jedem anderen klinisch verwendeten Opioid. Glattmuskeleffekte: erhebliche Bradykardie, Bronchokonstriktion, Chest-Wall-Rigidity. Vorwiegend in der Anästhesie/Intensivmedizin eingesetzt; Sufentanil-Nasenspray ist die einzige Formulierung mit eingeschränktem Einsatz außerhalb des OP (präklinische Analgesie).
