Tramadol ist ein atypisches Opioid mit dualem Wirkmechanismus. (1) Schwacher direkter μ-Opioid-Agonist (Ki ~2µM). Der aktive Metabolit O-Desmethyltramadol (via CYP2D6-Metabolisierung) ist ein potenter μ-Opioid-Agonist (Ki ~3nM, ~200× potenter als Tramadol). (2) SERT- und NET-Reuptake-Inhibitor (serotoninerge und noradrenerge Aktivität). Das duale Wirkprinzip macht Tramadol einzigartig unter Opioiden und erklärt sowohl die analgetische Wirkung als auch das erhebliche Serotonin-Syndrom-Risiko. Das CYP2D6-Polymorphismus bestimmt maßgeblich die Umwandlungsrate zum aktiven Metaboliten – langsame Metabolisierer haben schwächere Effekte, schnelle Metabolisierer höhere Toxizität.
