Morphin ist ein vollständiger Agonist an μ-Opioid-Rezeptoren (MOR). Die Gi-Protein-Kopplung hemmt die Adenylylcyclase (↓cAMP), aktiviert einwärtsgleichrichtende K+-Kanäle (Hyperpolarisation) und inhibiert spannungsgesteuerte Ca²+-Kanäle. Diese prä- und postsynaptischen Effekte führen zu Analgesie, Sedation, Euphorie und Atemdepression. Morphin zeigt auch geringere agonistische Aktivität an κ-Opioid- und δ-Opioid-Rezeptoren. Die μ-Opioid-Aktivierung im Nucleus accumbens und ventral tegmentalen Areal führt zum Belohnungseffekt und schneller Abhängigkeitsentwicklung.
